The Challenge
This diagnostic chain had grown from a single lab in 2018 to 12 labs across Delhi NCR by 2024. Growth brought scale but also complexity. Each lab had developed its own internal test codes and naming conventions, making it increasingly difficult to maintain standardised report formats across the network.
The problems compounding as the chain grew:
- SKU dissonance across labs: A "Thyroid Profile" at Lab 3 had a different internal code and report format than "Thyroid Function Test" at Lab 8 the same tests, different outputs. Cross-lab comparisons for patients were unreliable
- Manual report generation: Lab technicians were manually typing clinical narratives and reference ranges for each report a time-consuming process prone to copy-paste errors and outdated reference values
- Zero ABDM compliance: None of the lab reports were in FHIR R4 format, and no LOINC coding was applied making the chain non-compliant with ABDM's upcoming mandatory standards
- Patient confusion: Patients receiving reports from different labs in the chain could not reliably compare results. Phone calls to the lab for clarification were a daily operational overhead
- Insurance friction: Several TPA partners were struggling to auto-adjudicate claims because the lab's non-standard codes didn't map to their benefit catalogues. Manual mapping was causing claim delays
Why Smart Reports
After evaluating two LIS upgrade paths (both of which would have required replacing the existing Agappe system), the chain's leadership chose Zealthix's Smart Reports as a middleware layer that would sit above the existing LIS standardising output without disrupting the workflows that lab technicians had used for years.
Key selection factors:
- API integration with Agappe LIS without requiring data migration or workflow changes
- AI-assisted LOINC mapping that could handle the chain's 2,400 internal test codes in under two weeks
- FHIR R4 report generation that would meet ABDM requirements without additional development
- Patient-facing PDF reports with clear, jargon-free AI-generated narratives
The Deployment
Week 1–2: LOINC Mapping
Zealthix's AI mapped all 2,400 internal test codes across all 12 labs to their LOINC equivalents. The mapping engine achieved 96.2% automatic match accuracy the remaining 3.8% of codes were reviewed and confirmed by a senior pathologist on the client's team. The complete mapping was done in 11 days.
The LOINC mapping exercise also revealed that the chain had 340 duplicate test codes across its labs different internal codes for identical tests. These were consolidated during mapping, immediately reducing SKU dissonance across the network.
Week 3: Integration and Report Template Configuration
Smart Reports connected to the Agappe LIS via REST API. When a test result is finalised in Agappe, Smart Reports automatically pulls the result, applies the mapped LOINC code, retrieves the age/sex-specific reference ranges, generates the AI clinical narrative, and produces a FHIR R4-encoded report object alongside a patient-friendly PDF.
Week 4: ABDM Configuration and Go-Live
ABHA patient linking was activated at each lab's reception desk. All new patients are offered ABHA registration; existing patients are linked on their next visit. Reports for ABHA-linked patients are automatically posted to the ABDM ecosystem, giving patients and their doctors real-time access through any ABDM-compatible app.
Results at 60 Days
Measured Outcomes
The chain's TPA relationships also improved measurably. All three TPA partners that had previously flagged the chain's non-standard coding confirmed that claims were now being processed without manual intervention a meaningful reduction in days-outstanding for the lab's receivables.
Patient satisfaction feedback specifically mentioned the new report format as clearer and more useful. The AI-generated narrative which explains what each result means in plain language reduced the number of patients seeking immediate follow-up consultations just to understand their results.
What's Next
The chain is now expanding Smart Reports to include radiology reports from its three imaging centres applying the same LOINC-standardisation approach to DICOM-linked radiology output. They are also evaluating Zealthix's provider discovery platform to improve digital visibility across the network's lab locations.
